Your first stop as a patient will be at our Registration & Admitting Department, where we’ll welcome you, ask a few questions about your health history, and complete the necessary paperwork. The Admitting Department is just inside the main entrance to the left of the Information Desk. If you are coming in for Lab work and have registered with us in the past year, you may go directly to the Lab, no need to stop at Admitting. |

WHAT TO BRING: |
- Your health insurance card
- Your social security number
- A list of all medications you take (prescriptions and over-the-counter), along with the dosage
|

WILL YOU BE SPENDING THE NIGHT? HERE ARE SOME OTHER HELPFUL ITEMS: |
- Robe, slippers, pajamas
- Personal care items and toiletries
- Book or magazine
- List of family phone numbers in case you want to make phone calls
- Containers for eyeglasses, dentures, hearing aides, or other personal property
